REGISTER OF ANIMALS EXAMINED AND STAMPED FOR SLAUGHTER AT THE SLAUGHTER HOUSE

Name of cattle examin ed and stampe d for slaught er	Time and date of examination and stamping of the animal	Name and designation of the Officer who has examined and stamped the animal	The number and date of the certificate of examination	Time and date on which animal in brought to the slaughter house for slaughter	Time and date on which animal slaughte red	State whether the animal is slaughtere d within the prescribed hours after its examinati on	If the animal is not slaughtered within the prescribed hours state reasons for the same and state whether the animal is returned.
